The Mandatory Actions Unit (MAU) is the division within the California DMV that handles all DUI-related license suspensions, revocations, IID requirements, and reinstatements. Their phone number is (916) 657-6525, though they are notoriously difficult to reach by phone.

How long is the IID required?+
First offense: 6 months. Second offense: 1 year. Third offense: 2 years. Fourth+ offense: 3 years. The IID period starts AFTER installation, not after your suspension. Early removal is generally not permitted.
What is form DL-4006?+
DL-4006 is the "Application for Termination of Action" — the official form you submit to the MAU to request license reinstatement after completing all requirements. It requires proof of DUI program completion, SR-22 filing, IID compliance, and fee payment.
Can I get a restricted license during my suspension?+
For a first DUI offense, you may be eligible for a restricted license after 30 days of hard suspension. This allows driving to/from work and your DUI program. It requires SR-22 insurance, IID installation, and proof of DUI program enrollment.
What if I moved out of state during my suspension?+
You can request a termination of action using form DL-4062 if you’ve established residency in another state. You must provide proof of out-of-state residency, an SR-22, and affirm you won’t return to California for 3 years. The MAU reviews these on a case-by-case basis.
